Questions to ask potential New Doctor

We are looking for a new doctor. This would be our third one. The first one we NEVER saw.... we only saw the NP the entire 2 years we were going there. Current doc and office staff made a rather BIG mistake when giving us a refill Rx for my son's focalin.
